AI in Pharmaceutical Quality: What It Can Do, What It Cannot, and What Organisations Should Get Right

Artificial intelligence (AI) is already being used in the pharmaceutical industry in deviation management, document review, risk monitoring, visual inspection and regulatory submission workflows. The question is no longer whether AI has a place in quality, but rather how it can be implemented in a way that meets the robust governance and control requirements of GxP environments.

The opportunity is real. According to industry case studies and publications by the International Society for Pharmaceutical Engineering (ISPE), investigation cycle times can be reduced by 40–70% and effectiveness of Corrective and Preventive Action (CAPA) truly improved. Early adopters have demonstrated that AI can identify quality risks within fragmented data systems, enabling proactive intervention rather than reactive remediation. Although this data is largely based on industry reports rather than independent controlled studies, it consistently point in the same direction and can be used as a reference.

Nevertheless, there is also critical risks if AI is launched without proper validation, human oversight and integration with quality systems. This will result in non compliant documentation and vulnerabilities that will be exposed during an inspection, such as issues with data integrity, gaps in validation and failures in the audit trail. These problems are difficult to resolve afterwards and will invalidate a GxP study.

AI in the pharmaceutical industry should not be treated as a productivity tool added to existing quality systems, but rather as a regulated component within them. It should be qualified, monitored and governed as carefully as any other GxP critical process.

Quality Systems Create the Conditions for AI

To understand where AI adds value to quality, it helps to start with an honest assessment of where are the pain points in quality systems. The symptoms are very similar across organisations of all sizes:

  • Deviation investigations consume a disproportionate amount of time on documentation and formatting rather than root cause analysis. The analysis that should inform prevention efforts is crowded out by administrative activities that drive closure.
  • CAPA effectiveness is inconsistent. Recurring root causes because systemic patterns are not visible across fragmented data landscapes. The result is a CAPA system that closes records without solving problems.
  • Quality data is fragmented, QMS, LIMS, MES, SAP and supplier management systems each hold pieces of the quality picture, but there is no single system that connects them all. Decisions are made based on incomplete evidence because a full picture was never assembled.
  • Document management remains highly manual and subject to human error: controlled document reviews, version management, and SOP approvals create a substantial administrative burden. This not only consumes time but also diverts attention from content to process.
  • Most sites are reactive when it comes to inspection readiness. Inspection data is assembled manually, even though it should be available on a continuously updated dashboard. Preparation reveals gaps that continuous visibility would have addressed months earlier.

AI cannot solve poor process design. However, it can dramatically amplify the effectiveness of well-designed processes and surface patterns in data that human reviewers operating in isolation will consistently miss.

Where AI Is Already Delivering Value

The applications below are documented and verified, and are being actively adopted in the pharmaceutical industry. Rather than proofs of concept or vendor demonstrations, they are implementations with measurable outcomes, drawn from peer-reviewed sources, regulatory agency publications and industry case studies.

Deviation Investigation and CAPA Generation
This is the most mature and well documented application of AI in pharmaceutical quality. The structural challenge is well understood: deviation reports require investigators to review batch records, equipment logs, environmental data, historical deviations and procedural documentation before they can meaningfully assess the root cause. In practice, this process is slow and inconsistent, with too much emphasis on documentation rather than analysis.

Generative AI tools, including Microsoft Copilot implementations integrated into standard Office 365 environments, are being used to transform this workflow. This approach converts each section of a deviation or CAPA report into a set of focused questions. The investigator answers these questions, and the AI then assembles a structured, formatted, regulatory-compliant document. While the investigator’s judgment drives the content, the AI handles the documentation burden.

The results from early implementations are evident

  • investigation cycle times reduced from an average of 18 hours to approximately 3 hours in documented implementations.
  • Report writing time was reduced from 4 hours to approximately 30 minutes.
  • CAPA development time is reduced from 12 hours to around 2 hours.
  • Investigation cycle time reductions of 40–80% have been reported by multiple organisations.

Very important, these reductions do not come at the expense of analytical depth. Documented evidence consistently notes that investigators are freed up to focus on critical thinking rather than repetitive typing, which is precisely the shift that quality leaders should want. Documentation was not the value. The analysis is.

Nonetheless, the governance requirement is non-negotiable, all AI generated content has to be reviewed, approved and archived by qualified personnel before it can become part of official GxP documentation. The qualified personnel defines the process and remains the decision maker. No implementation changes that principle.

Predictive Risk Monitoring
Traditional risk management in pharmaceutical manufacturing is largely retrospective and periodic. FMEA assessments are conducted and risk registers are updated after events rather than before. Furthermore, the data connections that would enable the early identification of emerging risks are siloed within systems that do not communicate with each other.

AI-powered risk monitoring platforms address these issues by integrating data from QMS, LIMS, MES and environmental monitoring systems into a unified analytical environment, and then applying machine learning models to identify patterns that precede quality events. Consider, for example, a pharmaceutical manufacturer operating multiple sterile filling facilities. Following several minor observations relating to microbial contamination control, the company integrated an AI-powered risk monitoring platform into its QMS, environmental monitoring database, LIMS, and manufacturing execution system (MES). The AI system began aggregating data from environmental monitoring swabs, cleanroom air sampling, equipment maintenance logs and deviation reports, identifying correlations that the separate systems had not.

Document Review and Document Management
Controlled document management is resource intensive and, with effectively zero margin for error.
Natural language processing (NLP) tools are being used to speed up document review cycles, highlight inconsistencies between related documents, identify elements missing from regulatory requirements and generate draft responses to regulatory queries. AI tools specific to the pharmaceutical industry, which are trained on regulatory frameworks such as 21 CFR Part 11, ICH Q9 and EU GMP Annex 11, can review documents against compliance requirements with a consistency that manual review cannot match at scale.
This is particularly relevant for regulatory submission preparation. AI tools can review draft CTD sections for completeness against guideline requirements, highlight gaps in cross-referencing, and identify language inconsistencies that could generate regulatory questions. These capabilities become operationally paramount in the context of ICH M4Q(R2), which introduces explicit cross-referencing requirements between Module 2.3 and Module 3. Visual Inspection and Manufacturing Quality Control
Computer vision and deep learning systems are being used to inspect products at manufacturing line speeds, providing a level of consistency that human inspection cannot match. These systems are currently being used in active production of different type of products:

  • Film-coated tablet defect detection: Deep learning systems can identify coating defects at production line speed without the need for manual positioning, enabling consistent, high-volume inspection.
  • Printed character inspection: Systems can individually inspect hundreds of thousands of units for legibility and printing defects, significantly reducing false rejects while improving overall yield.
  • Parenteral product inspection: AI vision systems are increasingly used in the manufacture of sterile injectables, with manufacturers actively working with the FDA and EMA to ensure that their validation strategies align with GxP requirements.

Pharmacovigilance and Signal Detection
Pharmacovigilance relies on a volume of data that consistently exceeds the capacity for manual processing. In most pharmaceutical organisations, case processing activities consume up to two thirds of internal PV resources. Natural language processing (NLP) tools reduce this effort by extracting and classifying adverse event information from unstructured sources such as case narratives, medical literature, social media and electronic health records. Signal detection algorithms that identify emerging safety patterns across large datasets are supplementing traditional statistical disproportionality analysis.

The EMA, FDA and PMDA have all published guidance or position papers on AI in PV. The consistent message is that AI tools are acceptable in PV workflows, provided they are validated and their outputs are reviewed by qualified scientists, with continuous monitoring of performance.

What AI Cannot Do in GxP

An honest assessment of the use of AI in quality control requires equal clarity about its limitations and risks, because in a GxP environment, failure of AI systems can result in patient safety critical events or regulatory enforcement action.

AI Cannot Replace Qualified Personnel Judgment
Every verified implementation operates under human-in-the-loop oversight. The AI generates outputs. A qualified person then reviews and approves them, taking responsibility for it. This is not a transitional position pending further advances in AI, it is the permanent governance framework for GxP environments, and it reflects an accurate perception of professional accountability.

An AI system that identifies a probable root cause for a deviation offers a hypothesis based on pattern matching against historical data. The investigator who reviews that hypothesis brings context, professional judgement and regulatory accountability, which the AI system lacks and cannot acquire. AI is the tool that surfaces the hypothesis. A qualified person tests, analyses and owns it.

The practical expression of accountability is an SOP. Before deploying any AI tool in a GxP process, an organisation should have a documented procedure defining the scope of AI use, the required human review at each decision point, user training requirements, and the escalation path for questioning AI outputs. This applies to everyone involved in the process, regardless of their role, seniority, or technical familiarity with the tool.

In a GxP environment, the reliability of AI depends on the surrounding framework. An SOP does not limit AI. Instead, it establishes the foundation for consistent, auditable and defensible use by all those involved in a regulated process. The same standards that apply to any other controlled activity apply here: documented procedures, qualified users and clear accountability. These are not constraints to AI implementation. It is a prerequisite.

AI Trained on Poor Data Will Produce Poor Outputs
The quality of machine learning systems depends on the data they are trained on. In the context of pharmaceutical quality, this presents a particular risk: organisations with inconsistent, incomplete documentation or poor data hygiene in their QMS will develop AI systems that perpetuate and amplify these issues.

Preparing for AI deployment is primarily a project focused on data quality, rather than technology. Before using AI for quality GxP analysis, the input should be consistent, complete and accurately classified. Clearly, it is not simply a matter of clean data. It has also to be compliant. Training datasets must adhere to the ALCOA+ principles applicable to any GxP record. If an AI system is trained using data that does not meet these standards, it will produce non-compliant outputs.

Public generative AI tools are not suitable for GxP processes. Significant risks arise when data is entered and used to train public models, including non-compliance with data privacy regulations, exposure of intellectual property, and an inability to validate or audit how the model processes information. In order to use AI tools in regulated quality processes, they have to operate within a controlled, validated private environment to preserve data integrity requirements and prevent data from being shared beyond the boundaries of the organisation.

AI Cannot do without Validation
AI systems deployed in GxP environments are software used for regulated activities. They require validation under 21 CFR Part 11 and EU GMP Annex 11. The validation approach should address the specific characteristics of AI systems, such as the fact that machine learning models can change over time as they process new data, that outputs must be explainable and that performance must be continuously monitored.

Regulators expect users to be able to explain how an AI system reached a particular output, for instance, a root cause suggestion, a risk signal, a document gap. It will be difficult to defend the answer “the AI said so” to a regulator’s satisfaction during an inspection.

Both the FDA and the EMA have published their expectations. In January 2026, the two agencies jointly published a draft set of ten principles for good AI practice, signaling a convergent regulatory approach.

Quality managers who use commercial AI tools without verifying vendor validation documentation or fulfilling their own site specific validation obligations are creating compliance gaps that may be identified during inspections on both sides of the Atlantic.

AI Cannot Fix a Broken Quality Culture
AI deployment does not happen in isolation. It occurs within an organisation. If the wider organisation treats quality as a compliance documentation function rather than a risk management discipline, AI will continue to reproduce the same underlying problems, but faster. The tool reflects the system in which it operates.

Organisations that treat quality as a genuine and integrated risk management function, connecting GxP quality to operational and business decisions, will find that AI is a truly accelerator for operational excellence. The returns are proportional to the size of the investment rather than the sophistication of the tool.

The Regulatory Position Where Agencies Stand

Since 2023, regulatory agencies have transitioned from cautious observation papers to more active engagement, issuing joint principles and draft guidance with specific validation expectations. The direction is clear, even though the specific guidance frameworks are still evolving.

The most recent development is from January 2026, when the FDA and the EMA jointly published ten principles for good AI practice. This joint publication signals that regulatory expectations on both sides are converging. For organisations operating in multiple jurisdictions, this also represents a practical simplification: two regulatory regions, one set of expectations.

The main positions behind the joint draft principles:

  • EMA’s October 2024 Reflection Paper clearly established the European position: validation proportional to intended use, risk-based deployment and continuous monitoring. The EMA’s Quality Innovation Group has consistently advocated for structured adoption rather than blanket restriction. Pragmatic stance that reflects the current state of the industry.
  • FDA issued draft guidance in January 2025 setting out a risk-based credibility assessment framework for AI in drug and biological products. Anyone who has worked in software validation will be familiar with the core requirements: transparency, traceability, and lifecycle performance oversight. What is specific to AI, is the expectation that the validation framework will account for model drift, the tendency of machine learning systems to change their behaviour over time as they process new data.

The PMDA is also consistent with the FDA and EMA on the core requirements: validated tools, qualified oversight and monitored performance.

Considerations for Quality and Compliance Professionals

The regulatory framework is in place and the tools are available. The remaining decisions are practical and specific: where to start, how to build a governance framework that delivers value and how to ensure the implementation withstands regulatory scrutiny.

Define the intended use before selecting the tool

Start with the process in the quality system where the discrepancy between the effort invested and the value generated is greatest. Identify which, if any, AI-assisted tool is suited to that specific problem. Clearly defining the intended use drives tool selection, validation scope and governance design.

Treat deployment preparation as a data quality project
Map the data flows between the QMS, LIMS, MES, and any other systems that hold quality information. Identify where data is inconsistent, incomplete or inaccessible. Remember that an AI system is only as reliable as the data it operates on. In a GxP context, that data must adhere to the ALCOA+ principles before it should be used for training.

Establish the governance framework prior to implementation
Define the exact role of AI and the required human review at each decision point. To note that the SOP covering scope, user qualification, review requirements, and escalation paths is the document that makes deployment auditable and defensible.

Validate against the specific intended use
Commercial AI tools typically include vendor validation packages. These packages validate the tool in a generic environment. Site-specific validation must confirm that the tool performs as expected in the site environment, with the site-specific data, and for the intended use of the site.

Measure outcomes, not activity
The value of AI in pharmaceutical quality control should not be determined primarily by the number of reports generated or the time saved on documentation. Instead, it should be based on improvements in quality outcomes, for instance, fewer recurring deviations, more effective CAPAs and the earlier detection of risk signals. These outcome metrics should be defined prior to deployment and monitored over time.

Start with a targeted implementation, demonstrate value, then scale up
Begin with one site, one process with a clearly defined scope. Build capability and confidence before scaling up. Depending on the size and maturity of the organisation, deploying enterprise-wide AI within the first year may result in governance inconsistencies that exceeds the ability to be managed efficiently.
